Community Based & Individual Support

Community Based & Individual Support is where a person's goals turn into everyday habits-at home, in the neighborhood, and around town. We provide one-on-one assistance to build daily living skills, such as cooking, cleaning, money management, personal organization, and using transportation.

Support does not stop at the front door. We also help people explore local activities, visit stores and libraries, attend faith or social groups, and grow confidence in public spaces. Each visit is planned around the person's pace and comfort level, not a fixed program schedule.

Behind the scenes, we focus on dependable staffing, clear communication with families and Support Coordinators, and accurate billing within the person's New Jersey DDD budget. Our aim is simple: the supports written in the plan are the supports the person actually receives, so independence can grow steadily and safely over time.
